How to Register

Complete a registration packet for each student.

Packets are available online or at any Lynwood Unified school site.
Important: The packet must be completed before your registration appointment.

Enrollment Packet Letter for Parents (English and Spanish)
LUSD Student Enrollment Form (5 pages)
Student Emergency Card & Health Insurance Questionnaire (English and Spanish)
Title I Form
Home Language Survey (English/Spanish)
Report of Health Examination (English and Spanish)
Oral Health Assessment Letter & Form (English and Spanish)
Program Placement Options (English and Spanish)

For Families Seeking to Enroll in LUSD (Inter-District)

Inter-District permits allow families to request that their child attend a school outside their home district. If you would like your child to attend a Lynwood school but live outside of LUSD boundaries, you will need to:

Request an Inter-District permit from your current home district.
Once the permit is approved, submit it to the Student Services Department in the District Office or email it to dsandoval@mylusd.org.

For Families Seeking to Enroll in a School Outside of LUSD (Inter-District)

If you live within Lynwood Unified's attendance boundaries but wish for your child to attend a school outside the District, you must request an Inter-District Permit from LUSD:

Complete the LUSD Inter-District Permit.
Return it to the Student Services Department or email it to dsandoval@mylusd.org.

Request a School Transfer Within LUSD (Intra-District)

If your child is currently enrolled in a Lynwood Unified school and you would like to request a transfer to another school within our District, simply click the Transfer Request Online Form below to submit your request.

If the form is closed, families can make their request in person at the Student Services Department. Families will need to complete the Intra-District Permit Request form for in-person services.

Solicite un Cambio de Escuela Dentro de LUSD

Si su hijo(a) está inscrito actualmente en una escuela del Distrito Escolar de Lynwood y desea solicitar un cambio a otra escuela dentro de nuestro Distrito, simplemente haga clic en el Formulario de Solicitud de Transferencia en línea a continuación para enviar su solicitud.

Si el formulario está cerrado, las familias pueden hacer su solicitud en persona en el Departamento de Servicios Estudiantiles. Para recibir atención en persona, las familias tienen que completar el Formulario de Solicitud de Permiso Intra-Distrital.
